Check all the locks, or someone will break in. Don’t step on the crack, or someone you love will die. Don’t think that thought, or you’re a horrible person.

Our natural inclination as human beings is to avoid danger or at least figure out a way to manage that threat—even if the danger isn’t necessarily real. 

So...we avoid stepping on the cracks. We check and recheck the locks. We prepare for the worst at all times. Then, as usual, nothing bad actually happens, which only reinforces those coping mechanisms, further legitimizing their irrationality.

Recognizing that, Exposure and Response Prevention (ERP) therapy strives to starve the fear, uncertainty, and doubt at the heart of all the obsessive thoughts and compulsive rituals. Rather than discussing and addressing things solely on a cognitive level, ERP—though a gentle and gradual process—is about taking action in the real world to break the cycle once and for all.

Who Can Benefit The Most From Exposure And Response Prevention?

Many of the clients our ERP therapists see struggle with Obsessive Compulsive Disorder (OCD), which is a complex and largely misunderstood condition. Although TV shows and pop culture have illuminated aspects of OCD, it goes far beyond physical expressions, like excessive hand-washing or obsessive counting, portrayed in the media.

Some people don’t even realize they have OCD because no visible physical compulsions are occurring, when in reality they are mentally trying to find relief from the obsessions.

For instance, with magical thinking, obsessive fears, or even pedophilia OCD (POCD), the brain latches onto and can’t break free from constant, unwanted thoughts and imagery that cause immense distress.

While exploration and discussion are useful vehicles for getting us started, talk therapy alone can actually exacerbate OCD because, ultimately, we’re still feeding symptoms with attention. The same goes for our clients with long-standing anxiety issues, deep phobias, or complex trauma-related wounds that have left them hypervigilant or avoidant.

The goal of ERP is to give you concrete, actionable steps you can take to interrupt the cycle and develop healthier, more grounded ways of responding to fear and doubt.

Getting Started In Exposure And Response Prevention

The ERP course of treatment is a gradual and paced process that takes about 12 to 20 sessions on average to complete. To get started, we’ll first explore why healing and change matter to you while looking at what OCD, anxiety, or intrusive thoughts may have taken from you.

Next, we can talk about what you ultimately want to achieve, using all the insight we gain as motivators to ensure we never lose sight of what we are working toward. Then, we’ll create a hierarchy of challenges together—from least to most distressing—that you want to overcome or change through Exposure and Response Prevention.

How The ERP Treatment Process Works

If you’re like most people struggling with anxiety-related disorders like OCD, you probably experience a low tolerance for uncertainty. In fact, OCD is commonly referred to as the doubting disorder because so much of what drives deep-seated fears, catastrophic thinking, and compulsive behaviors is doubt and uncertainty.

In ERP, we target that uncertainty directly through Exposure, which involves you exposing yourself to an uncertainty that feels distressing. This process is different than outdated methods of immersion therapy in that we start small and build upward, so we will never jump straight into the deep end.

How Effective Is Exposure And Response Prevention Therapy?

ERP is a hands-on, evidence-based approach to treatment that has been extensively researched and tested. Often called the Gold Standard for treating OCD, Exposure and Response Prevention provides a systematic way of facing fears and interrupting the cycle of obsessions and compulsions.Compared to traditional talk therapy models, ERP offers real and tangible ways to confront OCD and anxiety that retrain your brain and help you start showing up in your life in new ways. ERP also works well in combination with other therapeutic methods. For instance, Mindfulness is a big part of ERP where, instead of calming the distress, we are trying to gently lean into it and sit with it until the fear and doubt essentially grow bored and “give up.” Rather than relying solely on skills, which can act more like temporary bandages than true healing, we’re working to enhance your emotional resilience and psychological flexibility in the long term.

How Is Exposure And Response Prevention Different From Other Therapy Models?

Gradually, you’ll develop a greater sense of agency and improve your relationship with your thoughts and feelings. What Inspired Us To Incorporate ERP Into Our Practice? 

OCD and anxiety disorders are much more common and misunderstood than most people realize. In fact, according to the International OCD Foundation, upwards of 75 to 80 percent of people with OCD are not getting diagnosed, with only 20 percent ever being referred to an ERP therapist for treatment.With such a huge number of people suffering and in need of real support, we felt it was imperative to make ERP a part of what we do at Key Counseling Group.
